1. On 01/31/2022, Title II ADA / Section 504 Coordinator(s) from the Office of Accessibility performed a Title II - Americans with Disabilities Act (ADA) Self-Evaluation. This evaluation was performed in accordance with 1991/2010 ADA Standards for Accessible Design.

2. Priorities for removing barriers are assigned in accordance with the ADA Checklist for Existing Facilities, promulgated by the ADA National Network. The checklist follows the four priorities listed in the Department of Justice ADA Title III regulations. These priorities are equally applicable to state and local government facilities:

Priority 1 - Accessible approach and entrance
Priority 2 - Access to goods and services
Priority 3 - Access to public toilet rooms
Priority 4 - Access to other items such as water fountains and public telephones

3. Corrective actions to findings were / will be submitted to the City maintenance work request system and will be managed in accordance with the City Transition Plan. Note: Recommended corrections may be modified where appropriate. Corrections are developed in consideration of the “Program Access” provisions of Title II, where applicable.

Surface cross slopes shall not exceed one unit vertical in 48 units horizontal (2-percent slope). When the slope in the direction of travel of any walk exceeds 1 unit vertical in 20 units horizontal (5-percent slope), it must be constructed as a ramp.

On the section of the accessible route along Haverhill Pl, there are 15 pads of concrete with excessive cross-slope ranging from 2.8% to 5.5%. This section also has excessive running slope but follows street grade and is therefore compliant. 12 pads are over 3%.

On this section of the accessible route along Haverhill Pl that runs from the turn to the playground north to the vehicle entrance, there are 10 out of 14 pads of concrete with excessive cross-slope ranging from 2.3% to 4.3%. 5 pads exceed 3% cross slope. This section also has excessive running slope but follows street grade and is therefore compliant.

Recommendation:

Remove and replace pads to compliance.

1/4 inch is the maximum vertical rise. Changes in level between 1/4 inch and 1/2 inch must be beveled at 1:2 or less.

As Built:

On the sidewalk that runs along Haverhill Pl. there is a 2" vertical displacement between pads. There was an attempt made to grind to a bevel, however, it remains a trip hazard as the ground area has a 31% running slope.

Recommendation:

Remove and replace both pads to compliance. The cost associated with one pad has been documented in a previous finding addressing cross slope.

There are four pads of concrete that comprise the vehicle entrance ramp along Havenhill Pl with a running slope of up to 15.1%. There is no level landing, the gutter pan to the ramp face is at least 1/2". The resulting cross slope along the public right-of-way sidewalk ranges from 7.4% to 15.2%.

Recommendation:

Remove and replace the curb ramp. If this is to remain a vehicular entrance curb ramp then it needs a level walk-around area.

The accessible route has running slopes greater than 5% and has not been constructed as a ramp (i.e. handrails,edge protection, etc.). Cross slopes are greater than 2% across the width of the walk.

As Built:

On the route into the park from Haverhill Pl. up to the playgrounds there are nine 9 pads of concrete with excessive cross-slope measuring between 2.4% and 4.2% (7 are over 3%). Much of the entire section of the route also has a high running slope measuring up to 10.3%.

Recommendation:

Explore changing this route due to the high running slope which may mean installing a longer route to the south end of Haverhill Pl. to achieve a compliant running slope. At the very least fix the pads with a high cross slope. Also, consider adding a curb ramp at this spot in order to provide an accessible route up to the playgrounds and the pavilion. See attached photo for an example.

Basketball Court- The surface of the pedestrian access route has vertical discontinuities greater than 1/2 inch or between 1/4 inch and 1/2 inch that are not beveled.

Vertical surface discontinuities shall be 0.5 inches maximum. Vertical surface discontinuities between 0.25 inches and 0.5 inches shall be beveled with a slope not steeper than 50 percent. The bevel shall be applied across the entire vertical surface discontinuity.

Vertical surface discontinuities shall be 13 mm (0.5 in) maximum. Vertical surface discontinuities between 6.4 mm (0.25 in) and 13 mm (0.5 in) shall be beveled with a slope not steeper than 50 percent. The bevel shall be applied across the entire vertical surface discontinuity.Advisory R302.7.2 Vertical Surface Discontinuities. The allowance for vertical surface discontinuities is for occasional expansion joints and objects such as utility covers, vault frames, and gratings that cannot be located in another portion of the sidewalk outside the pedestrian access route. However, objects such as utility covers, vault frames, and gratings should not be located on curb ramp runs, blended transitions, turning spaces, or gutter areas within the pedestrian access route. This may not always be possible in alterations, but should be avoided wherever possible. Vertical surface discontinuities between unit pavers should be minimized.

The basketball court has a 1/2" vertical displacement between pads with a 3/4" gap.

Recommendation:

Grind to less than 1/2" bevel. Fill the gap.
