1. On 04/21/2022, Title II ADA / Section 504 Coordinator(s) from the Office of Accessibility performed a Title II - Americans with Disabilities Act (ADA) Self-Evaluation. This evaluation was performed in accordance with 1991/2010 ADA Standards for Accessible Design.
2. Priorities for removing barriers are assigned in accordance with the ADA Checklist for Existing Facilities, promulgated by the ADA National Network. The checklist follows the four priorities listed in the Department of Justice ADA Title III regulations. These priorities are equally applicable to state and local government facilities:
Priority 1 - Accessible approach and entrance
Priority 2 - Access to goods and services
Priority 3 - Access to public toilet rooms
Priority 4 - Access to other items such as water fountains and public telephones
3. Corrective actions to findings were / will be submitted to the City maintenance work request system and will be managed in accordance with the City Transition Plan. Note: Recommended corrections may be modified where appropriate. Corrections are developed in consideration of the “Program Access” provisions of Title II, where applicable.

Turning Spaces A, B, and C have slopes of 3.8%, 2.9%, and 3.1% respectively. Connector 1 is comprised of four pads with running slopes between 5.4 and 5.9%. The two middle pads also have cross slopes up to 5.4%. Connector 2 has one pad with a cross slope of 2.6%. Connector 3 (four pads) has cross slopes between 3.0 and 4.3%. Additionally, there is a 3/4" vertical discontinuity between Turning Space C and Connector 3.
Recommendation:
Replace non-compliant turning spaces so that they have a less than 2% slope in all directions. Replace the connectors between these turning spaces so that they have cross slopes less than 2% and running slopes less than 5%.

There are both gaps greater than 1/2" wide and changes in elevation greater than 1/2" in the area of and surrounding the pavilion. Next to the pavilion, leading from the slides, there is a large divot in the concrete.

As Built:
The Cottonwood Creek Trail is a Shared Use Path that runs through the southeast side of Frank Castello Park. Per R302.5.4 Physical Constraints, "where compliance with ... R302.5.2 is not practicable due to existing terrain or ... similar existing physical constraints, compliance is required to the extent practicable." However, 31 concrete pads were assessed to have cross slopes in excess of 2%, ranging from 2.3 to 4.2%.
Recommendation:
When next rehabilitating this stretch of concrete, consider providing a distinguishable pedestrian pathway (patterned concrete?) either within or to one side of the clear width of the Shared Use Path. Ensure this pathway is compliant. This might especially be a consideration when trails intersect / travel through neighborhood parks where more / diverse traffic might be encountered.
